Information submitted through the inquiry form

The inquiry form requests basic contact information and a general description of the service, situation, and timing you want to discuss. The website delivers that information to a restricted PlannedWell Microsoft 365 shared mailbox through Microsoft Power Automate so an authorized team member can review and respond to it.

Information an initial inquiry should include

Provide only the general information requested by the form or needed to ask a general question. Do not include Social Security numbers, taxpayer identification numbers, account numbers, tax returns, tax notices, financial statements, passwords, or other confidential documents. PlannedWell will provide a separate secure method if documents are needed after the initial review.

Routine website information and analytics

Like most websites, hosting and security providers may process limited technical information such as an internet protocol address, browser or device type, requested pages, and the date and time of a request. This information may be used to deliver the website, maintain security, and diagnose technical problems.

PlannedWell uses Google Analytics to understand general website use, including pages viewed, approximate location, device and browser information, referral sources, link selections, guide downloads, and whether an inquiry form was started, encountered a validation error, or was successfully submitted. PlannedWell does not intentionally send Google the name, email address, telephone number, selections, or message entered in the inquiry form. Advertising personalization and Google Signals are disabled. The website does not include third-party advertising pixels or behaviorally targeted advertising.

How information may be used

Information from an inquiry may be used to respond, assess whether the requested work is within PlannedWell’s services and current capacity, arrange an introductory conversation, maintain appropriate business records, protect the website, and meet applicable professional or legal obligations.

Middlebrook Wealth and other professionals

PlannedWell Tax Advisors, LLC and Middlebrook Wealth are separate businesses. PlannedWell does not treat an inquiry as permission to share information with Middlebrook Wealth. Relevant information may be coordinated with Middlebrook Wealth or another professional only when appropriate and authorized, subject to the separate services and agreements involved.

Service providers and disclosure

PlannedWell may use providers that support website hosting, security, communications, document exchange, or other business operations. Those providers may process information only as needed to provide their services or as otherwise permitted by their agreements and applicable law. PlannedWell does not sell personal information or use website inquiry information for targeted advertising.

Retention and security

If no professional relationship begins, PlannedWell generally deletes inquiry information within 90 days after receipt. Information may be retained longer when reasonably needed for an applicable professional obligation, legal requirement, security investigation, dispute, or request from the person who submitted it. If a professional relationship begins, information is retained according to the applicable engagement terms and recordkeeping requirements. Reasonable safeguards can reduce risk, but no website, email system, or electronic transmission can be guaranteed completely secure.
